1 We hereby agree that our personal information given on the form will be placed in the company's database of CIJ and RPMG and will be used to organize training courses, seminars, conferences and other marketing activities in accordance with the Act of 29.08.97 r. on protection of personal data (Journal of Laws No. 133, item. 883). At the same time, I reserve the right to inspect and update the data contained.

1 We hereby agree to document the meeting in the form of photographs and / or video and to publish the above-mentioned documentation, as this contains our image, on websites and in printed materials of CIJ, RPMG and their trading partners.

1 We hereby accept the transfer, by CIJ VAT invoices in electronic form, in conjunction with Regulation of the Minister of Finance of 17 December 2010. On the transmission of invoices in electronic form, storage rules and procedures for sharing the tax authority or tax inspection authority (Dz. U. No 249, item. 1661). According to the findings the invoice will be sent from novakova@cijeurope.com to the address submitted in the form.
Withdrawal of acceptance of sending VAT invoices in electronic form may be in writing or by electronic mail to the email as above.

Participation fee per person: 480 EUR + VAT
The fee does not include hotel rooms.
Filing the registration form is an obligation to pay.
Confirmation of the registration will be e-mailed with a pro-forma invoice to the electronic address within 3 working days of receipt of the registration form.
In order to attend the Gala AWARDS 2018 the payment must be made in advance to the banking account shown in the pro-forma invoice.
VAT invoice will be issued and send via regular post to the address indicated by the attendee within seven days of receipt of the payment.
Should you cancel the registration prior to 14 days before the Gala AWARDS 2018, 100% of the payment made will be returned to you. If the cancellation is made between 7 and 13 days before the Gala AWARDS 2018, 50% will be returned. No cancellation will be accepted less than 6 days before the Gala AWARDS 2018.
If you do cancel your registration and do not attend, you will be charged the full amount of the registration fee.
The cancellation must be done in writing. No payment does not imply cancellation.
CIJ Europe and Roberts Publishing Media Group reserves the right to modify the program of the Gala AWARDS 2018 and under special circumstances to change its date and venue.
